Each family undergoes an initial assessment to determine the most appropriate services. 

Create a Care Plan

We pride ourselves on creating unique care plans that meet the individual needs of our clients.  

Match Caregiver to Family

Using our detailed assessment and experience, we match a qualified, compatible caregiver to each family.  Recurring background checks, drug-screenings and motor-vehicle inquiries are required for all of our caregivers.

Supervise

We regularly visit our families' homes to observe and advise our caregivers.  Supervision assures that you don't have to worry that your loved one is receiving the highest level of care available. 

Communicate With Family

Daily activity and care giving notes are provided for the families via our Axis Care Family Portal to keep informed of their loved one’s mental and physical condition as well as how caregiving is going. Supervisory staff regularly checks in via phone and email and is available to families any time.

Modify Care Plan

We reassess every client and adapt service plans to meet the changing needs at least every 90 days.  We do this by careful and considerate collaboration with families and the caregivers.
